We provide support to employees at every stage of their healing process.

The employer may receive notice of an injury from many sources, such as a nurse triage report, the injured employee, co-worker, supervisor, treating physician, or family member.

1. The employer completes lines 9-17 of the Workers’ Compensation Claim Form (DWC1) DWC-1 Employee Claim Form and gives the form to the employee to complete their portion.
2. The employer should have the employee sign and date the Acknowledgement of Receipt of Employee Claim Form.

3. If the employee completes the DWC-1, the employee is to receive a copy of this completed form. The employer MUST retain the original.
4. The appropriate supervisor should complete the Supervisors’ Report of Injury/Accident Report.
5. The employer will then submit to AIMS our Claims Administer for Workers Compensation.

SIG and our School District Members work together with the Norm Peterson and Associates Return to Work Program.

GOALS OF ERTW Program-

• Facilitate communication between employee, employer & physicians.

• Lessen the employee’s financial concerns, increase employee’s moral.

• Assist in early return to work within the employee’s physical abilities.

• Maintain productivity and decrease workers’ compensation costs.

• Increase Injured Workers’ physical capacity.

• Provide all concerned parties with a uniform approach to rehabilitation.
